Physical Activity Readiness Questionnaire (PAR-Q)

To ensure all athletes are in good physical health condition we need to collect the below Physical Activity Readiness information.

Please be honest in your answers to ensure you are ready to meet the physical requirements your training program will require.

If you answered YES to one or more of the above questions, talk with your doctor BEFORE you start becoming much more physically active or BEFORE you start a coaching program. Tell your doctor about the PAR-Q and which questions you answered YES to. You may be able to do any activity you want as long as you start slowly and build up gradually. Or, you may need to restrict your activities to those which are safe for you. Talk with your doctor about the kinds of activities you wish to participate in and follow his/her advice.

If you answered NO to all the PAR-Q questions, you can be reasonably sure that you can start becoming more physically active and start a coaching program. It is also highly recommended that you have your blood pressure evaluated. If your reading is over 140/90, talk with your doctor before you start becoming more physically active or starting a coaching program.
